I am looking for a beauty therapy disclaimer. Any advice?

I am running reasonably sized charity event on behalf of cancer backup and breast cancer care next week. This event involved a range of beauticians offering a variety of therapies including manicures, facials, and holistic treatments to members of the public for a donation to the charities. I am looking to have a generic disclaimer which all the participants will sign on registration to the event. This will just say that the therapists (who are all fully qualified) aren't liable for anything. Does anyone know of any links online where I can print off such a disclaimer, or do you have any suggestions on how to word it? Many thanksI am looking for a beauty therapy disclaimer. Any advice?
The undersigned understands that the event does not gauarantee to make you prettier.

The undersigned idemifies the Promoter, Participants, the rude lady in line and anyone currently in the USA including international air space from the following

Injury

Hair loss

burns, scars cuts, stinky solution

death, dismemberment, tooth loss, bad breath.

Falling building parts, sagging body parts, and sore feet or the theft of the above.

Also this agreement will cover anything else the Promoter has failed to mention, such as the loss of psychic abilites.

I would hire someone if I where that concerned. a generic disclaimer would provide fodder for any attorney.
